In the event the ordained person is unable to locate his/her credentials for surrender, he/she shall sign a declaration that said credentials are lost and acknowledges that he/she is no longer authorized to exercise ordained ministerial functions. This declaration shall be given to the district superintendent for deposit with the secretary of the conference.
