Pastor(s) administering the Sacrament of Baptism to an infant or child of divorced parents shall carefully ascertain that both parents agree to the action. The parents' written statements of agreement shall be kept on file with the baptismal records. If a parent has sole legal custody of the infant or child, or if one parent cannot be located for agreement, the parent desiring the sacrament for his or her infant or child shall make a written statement of the situation, which shall be kept on file with baptismal records.
